Three items are required for your child's health file at school. They are:

  • Immunization Records, translated into English
  • Student Health History Form
  • Confidential Medical Emergency Record and Permission form

In addition, if your child will participate in school-sponsored sports, an annual physical exam is also required. A current physical must be on file in the nurse's office before your child can participate in practice or competition.

 

Physical Exams

  • A complete School Physical by a healthcare provider is highly recommended if your child is entering kindergarten at Brussels American School. The physical may be completed at any medical facility. Please bring a copy of the physical to the school.
  • If your child plays school-sports, then a physical is required once every calendar year while they are attending school at BAS and are involved in sports.
  • The School/Sports Physical can be done at the NATO/SHAPE Brussels Health Clinic, which is on the school grounds at BAS. Any student who attends BAS is eligible to have these health requirements completed at no cost, at the NATO/SHAPE Brussels Health Clinic. An appointment can be made at the clinic by calling +322-717-9500.

Before enrolling in a Department of Defense school, students are required to have the following immunizations listed below. These requirements represent the minimum requirement and do not necessarily reflect the optimal immunization status for a student.

DoD 1342.6M allows parents TEN DAYS to provide the school with documentation satisfying these requirements, prior to disenrolling the student, until proof of the necessary immunizations is provided to the principal.

BRING YOUR CHILD'S IMMUNIZATION RECORDS with you (translated into English by your medical authority), when you come for registration. Without this document registration cannot be completed.

The requirements are:

  • DTP (Diphtheria, Tetanus, Pertussis ): 4 doses, at least one of which was administered after the 4th birthday.
  • TdaP Booster (Tetanus, diphtheria, adolescent Pertussis) : at age 11-12
  • HIB (Haemophilus influenza type b): 2-4 doses, not routinely given to children 5 years and older.
  • Hepatitis A: Two doses, administered at least six months apart.
  • Hepatitis B: 3 dose series.
  • MMR (Measles, Mumps, Rubella): 2 doses.
  • IPV/OPV (Polio): 3 doses, at least one dose must be administered after the fourth birthday.
  • Varicella (Chicken Pox): one dose through the age of 12 years, 2 doses for those 13 and older (at least one month apart) or reliable history of the disease,(please provide date).
  • Meningococcal MCV4 (Meningococcal Meningitis): one dose age 11-12, as well as unvaccinated adolescents at high school entry (15 years of age).
  • TB skin test (Tuberculosis): The NATO Health Clinic physicians will determine if this is necessary for your child at the time of their physical exam.

Medication at School

If your child needs to take non-prescription or prescription medications during the school day, a Medication During School Hours form must be completed and signed by you and your child's healthcare provider, and renewed every school year. Please read this form carefully, print and fill it out.

Medical Power of Attorney

If you are a single parent going out of town, or if both parents are going out of town, and your child is staying behind, you will need to designate someone to have Medical Power of Attorney for him/her while you are away. Please fill out the attached form, give one copy to whom you have designated to have power of attorney, and one copy to the Health Office at school.
